DESCRIPTION:

The Washington County Health Department's HIV Case Management Program is seeking a full-time contractual Community Health Nurse II. This position will provide clinical case management services to persons in Washington County who have been diagnosed with HIV + or are diagnosed with AIDS. The job responsibilities include: complete Bio-psychosocial assessment and all required paperwork within 7 days from the date case is open; perform annual care plan and 6 month updated care plan on all consumers within timeframe guidelines; monitor consumer immunization status, collaborate with PCP/HIV specialist and consumer to provide immunization within annual guideline including PAP, emergency care visits; provide direct clinical Case Management services to a caseload of persons; perform nursing/case management duties as required for HIV medical clinic; attend monthly multidiscipline care plan meeting, program team meetings, supervision, etc.; liaison with community physicians; and respond to consumer phone calls.

MINIMUM QUALIFICATIONS:

EDUCATION:

A Bachelor's Degree in Nursing from an accredited college or university
|
EXPERIENCE:

None
NOTE: A Bachelor's Degree in a related field and one year of experience as a Registered Nurse may be substituted for the required education. Two years of experience as a Registered Nurse may be substituted for the required education.

LICENSE:
|
Candidates in this classification must possess a current license as a Registered Nurse from the Maryland Board of Nursing.
